FAQ

Need to know more about our services?

Community First Services and Supports (CFSS) is a Minnesota program for people with disabilities or chronic conditions. It helps them live more independently by giving them more choice and control over the services and supports they receive. CFSS replaces Minnesota's PCA program.

In the Agency Model, Headway Care is your support worker's employer. We hire, train, supervise, and pay your worker, and we handle payroll, taxes, and compliance. You still have a voice: you can recommend someone you know, including some family members, and help decide how your care is provided.

To receive CFSS, a person usually needs to:

  • Be eligible for Minnesota Medical Assistance (MA) or a qualifying waiver
  • Live in the community, not in a hospital or nursing facility
  • Have an assessment (MnCHOICES) showing they need help with daily activities

Both children and adults can qualify. If you're not sure whether you're eligible, contact us and we'll help you find out.

  1. Contact Headway Care at 612-443-8934 or Info@headwaycaremn.com.
  2. If you don't already have a MnCHOICES assessment, request one through your county or health plan.
  3. Choose Headway Care as your provider and let your case manager know.
  4. We'll work with you and your case manager to build a care plan and match you with a caregiver.
